We are able to provide help for you or your loved one when it comes to life-threatening or end of life needs, palliative or hospice care.

How does end of life care work?

Most of the people we care for want to stay in their own homes, so we help them do just that. At times when this is not possible or when a patient prefers a clinical environment, we can also support the hospital with our specialist care nursing staff.

Our nurses and healthcare assistants are trained to provide the utmost in care whatever the circumstance, providing a full range of complementary therapies and treatments to ease these challenging circumstances. 

Our number one concern is the emotional, psychological, spiritual and social needs of our patients, families, children and hospice carers. We aim to make life as comfortable as possible for everyone involved during this difficult period.

What is live-in care?

We know people are often looking for a real alternative to a residential or nursing home and so we provide live-in care that’s 100% tailored to you and your loved one’s needs.

Our carers will live at home with you or your loved one so they can provide around-the-clock support, which can be so reassuring for those with learning disabilities, elderly relatives, or those with dementia, Parkinson’s, Alzheimer’s or other debilitating illnesses.

It provides a real (and affordable) alternative to a residential or nursing home and provides you with real peace of mind.

How does live-in care work?

We know that this type of care requires the utmost sensitivity as our caregiver will be in your own home long term.

We know you’ll have plenty of questions you’ll want to ask, so we ensure there’s an initial informal and free of charge meeting with one of our experienced nurse managers to explain everything and allay any fears you may have. She will meet with everyone involved, including (and most importantly) the individual requiring care.

Our nursing manager will then draw up a care plan based on your requirements, offer a selection and rota of appropriate caregivers which suit your requirements and create instructions for them. 

We provide care on your terms, creating a unique care plan for you or your loved one. This plan will be designed to ensure we’re meeting all your needs including meals and eating times, naps and hobbies, dressing options and routines as well as around medication. As your needs change, so our one to one care can evolve with you.

For long term care, we will rota a group of carers so you get a good quality of continuing care - again, suited to you or your loved ones’ needs and values. 

During the initial two weeks’ of care, our nurse manager will be in regular contact to ensure you’re 100% happy with the help you’re receiving.

What are domestic services?

Doing things around the house can provide a real challenge for some of our loved ones but RealCare+ knows how important these little things are for them and their wellbeing.

Domestic services incorporate anything we can do to help make your loved one’s life at home better which don’t require professional or medical expertise, but still need to be delivered with attention, care and love:-

- Cleaning and help around the house (including hoovering, dusting, washing up, laundry, ironing and general cleaning)

- Food preparation and purchases 

- Some life admin tasks (subject to certain limitations which will be explained thoroughly in our initial interview)

- Transport and getting around 

All these services will be provided by the very best in care-providers hand-selected by RealCare+ to ensure your peace of mind.

How much do personal care services cost?

We can provide domestic services for as little as 2 hours a week to 24/7 support. These services are tailored to your needs and fees are comparable and affordable.

Learning disabilities

Our aim is to ensure that those we care for with learning disabilities are able to live their best life.

What care is provided for those with learning disabilities?

We support you with highly trained nurses, experienced in and possessing the best skills necessary to assist people with learning disabilities, autism and severe challenging or complex behaviour.

We work in partnership with you to ensure their quality of life, helping them to socialise, learn, work and integrate with society. This naturally requires a highly flexible approach to both the physical environment and the care necessary, so we are experienced in working in various environments including the home, private rental property or housing associations.

This highly specialist area of nursing requires staff training which we invest in to ensure our team are experts and can carry out or respond to any activities. As well as mandatory and statutory training, we provide the following specialisms:-

- Challenging behaviour

- Autism

- Dyslexia

- Epilepsy

  • - Rectal diazepam and buccal midazolam administration
  • - Gastronomy feeding (Peg feed)
  • - Aspergers
  • - ADHD
  • - Tracheostomy

Our staff operate a system of person-centred planning and robust risk assessment, so we can guarantee your loved one is able to access opportunities and have fun. Ensuring consistency in the delivery of the behavioural plan is vital therefore to reduce any unnecessary incidents.
